Routt County Individualized Service and Support Team

Organization Name: Routt County Individualized Service and Support Team (ISST)

Agency Description: The Individualized Service and Support Team (ISST) is a family-driven, strengths-based, solution-focused problem solving process to support the youth (ages 0-21) in Routt County. It is a multi-disciplinary group of professionals that meet with youth and their families to assist them in identifying, finding, and accessing resources and services. With everyone together in one meeting, a unified plan is created and everyone is on the same page at the same time, which really simplifies things.
A brainstorming process occurs that targets the goals of the youth and family; an action plan is developed in which specific items and timeframes are agreed upon to provide services and allocate resources for the youth/family. An example of the broad range of ISST resources that have been provided in the past include things such as academic tutoring, counseling support, employment skills training, internship placement, scholarships for extra curricular activities, after school programming, mentoring partner, interpreter, art lessons, summer activities, medication, wheelchair lift, etc.

REPS is a largely volunteer 501(c)3 coalition of community members invested in stopping suicide in the Yampa Valley and supporting those who have lost a loved one to suicide through education and awareness. We believe that suicide is devastating to our community and that there are alternatives. Our programs include a 24/7 hotline, the Suicide Prevention Advocates program at Yampa Valley Medical Center, ASIST and QPR training, a Depression/Bi-Polar Support Group and a Heartbeat Survivor Support Group. We also do many trainings in our schools and offer one-on-one crisis support. Through generous grants we have money available to help pay for therapy for those who need assistance.

Main goal: To develop environmental learning opportunities for residents and guests of NW Colorado. Established in 1992 Yampatika continues to be the only non-profit organization that offers a continuum Yampatika-Photo2of educational services for ages preK through senior citizen in Routt, Moffat & Jackson Counties.

Yampatika operates an Environmental Learning Center at the City of Steamboat Springs-owned Legacy Ranch. The historical ranch is a rural backdrop for cutting edge science programming for youth (incl. at risk), adults & families with camps, school programs, self-guided tours, and snowshoe adventures.

Yampatika is part of a statewide and national movement to create systemic change in schools with our Environmental Literacy Program serving K-5th grades.

Each year we serve 15,000 with safe, enjoyable, and educational outdoor activities that instill a sense of environmental stewardship.
