Routt County Individualized Service and Support Team

Organization Name: Routt County Individualized Service and Support Team (ISST)

Agency Description: The Individualized Service and Support Team (ISST) is a family-driven, strengths-based, solution-focused problem solving process to support the youth (ages 0-21) in Routt County. It is a multi-disciplinary group of professionals that meet with youth and their families to assist them in identifying, finding, and accessing resources and services. With everyone together in one meeting, a unified plan is created and everyone is on the same page at the same time, which really simplifies things.
A brainstorming process occurs that targets the goals of the youth and family; an action plan is developed in which specific items and timeframes are agreed upon to provide services and allocate resources for the youth/family. An example of the broad range of ISST resources that have been provided in the past include things such as academic tutoring, counseling support, employment skills training, internship placement, scholarships for extra curricular activities, after school programming, mentoring partner, interpreter, art lessons, summer activities, medication, wheelchair lift, etc.

NWRM CASA provides advocacy services for abused and neglected children in Moffat, Routt and Grand Counties. CASAs (Court Appointed Special Advocates) are volunteers who are screened and trained to work with children and families in the child welfare court systems. Their primary role is to assure these children’s emotional, physical, educational and mental health needs are met and to help assure they are placed in a safe permanent home. CASAs are not professionals but are individuals who are passionate about children and want to make a difference in their lives. CASAs are assigned to a child by a Judge. The CASA meets with the child regularly and also gathers information from the family, foster parents, counselors, teachers etc, and provides the information to the Judge. This allows the Judge to have adequate information in order to make good decisions in the child’s best interests.
